Hospice & Palliative Care Nurse Practitioner – Up to $135K /yr

Are you a compassionate and experienced Nurse Practitioner with a passion for hospice and palliative care? We are seeking a skilled and empathetic Nurse Practitioner to provide high-quality, patient-centered care in a hospice and palliative setting. This full-time role is ideal for a professional who thrives in a collaborative environment and is committed to delivering excellence in end-of-life care.

Responsibilities

  • Conduct comprehensive assessments of patients and develop individualized care plans.
  • Provide direct patient care, including symptom management and pain control.
  • Collaborate with interdisciplinary team members including physicians, nurses, social workers, and chaplains.
  • Educate patients and families about disease progression, treatment options, and end-of-life care.
  • Participate in care conferences and contribute to ongoing care planning.
  • Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
  • Provide support and guidance to staff regarding clinical issues and best practices.
  • Participate in quality improvement initiatives and staff education programs.
  • Ensure compliance with all applicable laws, regulations, and standards of practice.

 

Job Requirements

Requirements
Graduation from an accredited School of Nursing and Nurse Practitioner Program.
Current nursing licensure in Hawaii and valid BLS certification.
Master’s degree in Nursing.
Minimum of 1 year of Nurse Practitioner experience and 1 year of hospice or palliative care experience.
Accreditation from an approved certifying body for advanced practice nursing as required by the state.
Preferred certification in a specialty area (e.g., CHPN, Pediatrics, Geriatrics).
Must possess a valid driver’s license and have access to a reliable, insured vehicle.
